yProcess for Filing for Lump-sum Withdrawalz

Before leaving Japan, obtain the claim form, gRequest of Arbitration for Lump-sum Withdrawal Benefith (Dattai ichijikin saitei seikyu-sho) from the Social Insurance Office (shakai hoken jimusho) or the National Pension Section of the municipal office nearest you or download it above.

 

Important Note:

If you intend to file for a refund on the 20% tax that will be taken from the Lump-sum Withdrawal upon payment, you MUST designate a tax representative to handle matters on your behalf before you leave Japan.

 

Fill in the necessary information (see note below). You will need to know the information included in your blue Pension Book, i.e. basic pension number. Mail the completed form and accompanying documents to the Social Insurance Agency after leaving Japan. Keep in mind that the claim must be made within two years of leaving Japan. You must attach the following items to your claim form:

 

1. Pension Book (the book will be returned to you at a later date. If you do not have your pension book, be sure to fill in all required information in the appropriate section of the claim form.)

2. A photocopy of your passport (page(s) showing your name, date of birth, nationality, signature, work visa, and the date of departure from Japan)

3. A document (copy of your bank statement, bank passbook, etc.) verifying the bank name, branch office name, branch address, bank account number, and the full name of the account holder (must be your own). Make sure that you have the correct bank stamp before sending it.

 

Note:

The Claim Form allows for four different types of pensions numbers, but ALL JET Programme participants need only to supply the "Basic Pension Number". The other three types of pension numbers can be left blank. Please mail the claim form along with the accompanying documents to:
